Amazon Canada Associates have the ability to schedule an accommodation assessment in the event they are experiencing symptoms or dysfunctions that would limit or restrict their ability to perform normal duties.
The accommodation assessment involves a virtual (video) session with a health provider where an evaluation of the symptoms are performed and modifications to the duties are recommended. Alternatively, associates can download the same forms, visit a clinic or family physician and have the forms completed.
- Amazon Associates will schedule a virtual session with Phzio or download the RFI form and take it to a clinic. Note: If you choose to have clinical physician complete your RFI froms
- There is no cost with the accommodation assessment through Phzio. If you visit a clinic or family physician you may be charged a fee.
- You can typically see a Phzio provider within 24hr during week days.
